Go through your Congressmans Office. *They have people who's job it is to take care of such things.
My wife got hers in a week with the aid of our Honorable Congressman Howard Coble. And it didn't cost any extra. ;) They do it for the votes and it works, my wife has voted for him every time since then even though he is not of her party.
