yeah paint the wood and the deck first then put together. i'd go with mas epoxy to bond the 2, use the slow so you have plenty of time to work it plus better stronger bond. yeah use the 1/2 most ply you can find plywood. the best plain is cabinet grade hardwood ply from lowes as it is 9 to 11 ply in 1/2 inch. get plenty of stuff for weight to set on it to hold tight to the floor till dry. several 5 gal buckets full of water or sand works great for weight. make sure you grind the back of the floor all over to get a good bond to clean glass.

Should I put any layer of glass between the wood and the cap? or just epoxy?

just epoxy will work as long as you grind the old glass surface to get a good bond.
